Luana Neves Ribeiro died after a bone marrow donation procedure, a simple procedure where bone marrow is extracted from the inside of bones. But Dr. Flávia Leite Souza Santos inserted a catheter through the jugular vein, which passes through the neck.
Commonly, bone marrow is collected from the donor's pelvis, not the neck.
It was Érika Rodrigues Pontes who was responsible for attending to the student on her return to the hospital.
The report from the Death Verification Service (SVO) of the Rio Preto School of Medicine (Famerp) attests that the young woman had her subclavian vein perforated, which caused hemorrhage and hypovolemic shock - a drop in blood pressure caused by massive blood loss - which led to her death. The perforations are said to have occurred during the attempt to implant a catheter to collect bone marrow in the left subclavian vein (near the jugular).
The document shows that there were "multiple perforations in the left subclavian veins" as the basic cause of death. Consequently, intrathoracic hemorrhages occurred, which caused hypovolemic shock. "Hypovolemic shock results from the loss of blood or plasma volume. This can be caused by hemorrhage, fluid loss, or trauma, with shock being a dysfunction that, if not corrected, leads to death," says the report.
The Biomedical portal indicates that the young woman spent an hour agonizing, waiting for a doctor. Luana was donating bone marrow to cure a child with leukemia.

What is the transplant like for the donor?
Before donation, the donor undergoes a rigorous clinical examination, including complementary tests to confirm their good health. There is no requirement for changes in lifestyle, work, or diet. The donation is performed in an operating room, under anesthesia, and lasts approximately two hours. Multiple punctures are made with needles into the posterior pelvic bones, and the marrow is aspirated. A maximum of 15% of the donor's marrow is removed. This removal does not cause any health compromise.

Detective João Lafayete Sanches Fernandes opened an inquiry yesterday to investigate the circumstances of the death of university student Luana Neves Ribeiro, 21. He sent an official letter to the HB requesting urgent information about the patient's clinical history and the names of all healthcare professionals involved in her care, as well as the directors of the HB and the Bone Marrow Transplant Unit. "I want to start the testimonies as soon as possible."
The Hospital did not report the death to the police!!!
Lafayete said he will question why the police were not notified of the young woman's death. The inquiry was initiated by the detective himself when he learned of the tragic episode solely through the press. "It was not a common death. The donor was fine, in perfect health, but died shortly after the procedure. Doctors missed chances to save Luana's life
Even after perforating the student Luana Ribeiro's left lung with a catheter, doctors at the Base Hospital had at least three opportunities to save her life.
According to doctors from the hospital itself, she should never have been released given the difficulty the professionals had in passing the catheter and should have remained under observation for at least 12 hours.
"The big mistake was releasing the donor. If she had been admitted, she would certainly be alive today," said a doctor who asked not to be identified. Another missed opportunity was to send her to the emergency room.
Upon arriving at the hospital with severe abdominal pain, the student was treated in the transplant department. In the emergency room, the chances of diagnosing the hemorrhage would have been certain.
The third opportunity was lost during Luana's treatment when she complained of pain. Dr. Érika Rodrigues Pontes, who is part of the HB's bone marrow transplant sector, should have suspected that Luana's pain was a consequence of complications from the catheter insertion instead of prescribing medication for nausea and leaving.
